@mattedgar Probably A. I have definitely done C in the past. But I think the right thing to do if you have time is A + D (email them a list of the problems you faced, how that might exclude others and a couple of links to their legal duties).

What happens to you if you don't fill in the form?
If someone has significant power over me with such a form (i.e. it's the UK gov) I grit my teeth and fill it in as best as possible.
If I can cope with being retaliated against I'll complain after.
If they don't have significant power over but there's a deadline, I'd send a plain text response written out in a separate Word doc or email.
